Requirements
- Two (2) photocopies each birth & baptismal certificates
- Three (3) copies of 1×1 ID picture
- Certified true copy of latest grades with an academic average of at least 80% or higher
- Must have a conduct grade not lower than 82%
- Two (2) recommendation letters from the school authorities (for high school applicants)
- 1 long folder
- 1 long brown envelope
- Testing fee of Php 300.00
